VaccineRequirements
Diphtheria, tetanus, and pertussis vaccine (DPT)
  • PK3 / PK4: 3 or 4 doses
  • Kindergarten through 12th Grade: 4 or 5doses
Polio Vaccine
  • PK3 / PK4: 3 or 4 doses
  • Kindergarten through 12th Grade: 4 or 5 doses*
  •  *If the fourth dose of polio vaccine is administered prior to the fourth birthday, a fifth dose of polio vaccine is required for kindergarten entry only.  
Measles, mumps, and rubella vaccine (MMR)
  • PK3 / PK4: 1 dose
  • Kindergarten through 12th Grade: 2 doses
Hepatitis B vaccine
  • PK3 /PK4 : 2 or 3 doses 
  • Kg through 12th Grade:3 doses
Varicella vaccine
  • PK3 / PK4: 1 dose
  • Two doses:Kindergarten and Grades one through eight.
  • One dose: Grades nine through twelve+
  • +Varicella Vaccine is not required if there is a history of Varicella disease documented by the healthcare provider in the space provided on the DH 680 form,the student's Florida Certificate of Immunization.
Haemophilus influenzae Type b (Hib) vaccine
  • PK3 / PK4: 3 or 4 doses
Tetanus-diphtheria-pertussis booster ( Tdap)
  • For all students attending 7th grade in the State of Florida, an updated FL Form 680 showing one dose of Tdap must be submitted to the school where they will be attending 7th grade.  All new or transferring students to grades 7-12th grade must also show proof of 1 dose of Tdap vaccine documented on a FL Form 680.
